The Shoreline Polio Group was formed in 2010 to provide support and information on resources to those who are now dealing with Post-Polio Syndrome.The members of the group live along the Connecticut and Rhode Island shoreline. Membership is open to anyone afflicted by polio or their family members.Our group meets in the Rhode Island area throughout the year. Visit our About Us page to see our schedule. Go Packet

A Go Packet contains information about your health conditions and what you do on a regular basis to maintain your life. It is designed to provide care givers information about your needs should you be hospitalized.Some suggestions to add to your packet
Include your name and date of birth
Do you have other health conditions?
Do you follow a special diet?
Do you have food allergies?
What medications do you take?
Do you have a daily exercise routine? Use a heating pad?
Are you cold sensitive?
Do you have sleep requirements, extra blankets, pillows, Cpap?
Do you require anything to support breathing?
Do need mobility assistance, braces, canes, scooter, shoes?
Do you need help with ADLs?
Do you have physical limitations? walking, weak limbs, balance, etc
Do you have vision & hearing issues?
How does Post Polio Syndrome affect you?
